"Quick Buy" in modal window. Fields: product, name, email, phone, message, send-button. Sending data (orders) to an email.

Hi there.

I want to have a quick buy thing in my web-shop. There will be a button in product grid/list which is called "buy in one click".
When customer clicks on the button there, a modal window should appear offering to fill a small form with fields like "product link (hidden), name, email, phone, message and a send button". When the form is submitted the info should be sent to my email. In the coming email I have to see which product is wanted (product link).

I want you to help me with the form only. I will pay. Will style the form myself. I want the code which will make the form work.
