Changes to Contact Us Page

We use our Contact Us page for customers to request quotes from us.  Currently, the page only has the name and email address fields on it. We would like to add a field for company name and phone number and make these fields required to submit a quote.  Any help would be greatly appreciated.

Hi,

I am also wondering how to insert my company details on the contact page also. I am using ver 1.60

Anyone can help?

Hi,

My contact page display 2 small boxes follow by a message box below. How do I add the word Name for the 1st box, email address for the 2nd box, and message for the 3rd box.
