Customize Cart Appearance

I'm looking for two changes to the cart:

[1] The In-Store Pickup option displays our stores as a drop-down.  Radio buttons would work better.  Plus, I'd rather the address not show, just the name of each store (there are 2).

[2] A Cart Attribute was created (text area) for Order Notes.  This appears as a row by itself on the cart page.  It is a space hog.  I'd like it moved down next to the Shipping Estimate, where the Discount Coupon used to appear before I disable that section.
