4.0 error when configuring Tax Provider Manual (Fixed or By Country/State/Zip)

With theTax Provider Manual (Fixed or By Country/State/Zip) installed (and using a DB upgraded from 3.4 straight to 4.0 using all upgrade scripts, when browsing from the admin dashboard to tax -> tax providers -> (Manual (Fixed or By Country/State/Zip)) configure (http://mysite/Admin/FixedOrByCountryStateZip/Configure) I receive an error. In the log this is:

I have checked and dbo.TaxRate is not a table in my database on either 3.4 or 4.0, but doing a text search for that in VS also returns no results...

The work around seems to be to take note of all of the tax rates (yes they are still correct even though there is no table?!), reinstall the plugin and redo the tax rates. I am guessing this is due to different plugins being merged sometime between 3.4 and 4.0. It would be better if the plugin just created the table if it doesn't exist, but there you go...

Combine "Nop.Plugin.Tax.CountryStateZip" and "Nop.Plugin.Tax.FixedRate" plugins into single one. Upgrade instructions: do not forget to manually delete "/Plugins/Tax.FixedRate" and "/Plugins/Tax.CountryStateZip" directories (they are not used anymore) and replace the plugin «Tax.Fixed» or «Tax.CountryStateZip» by the plugin «Tax.FixedOrByCountryStateZip» in the App_Data/InstalledPlugins.txt file.

Of course, that was carried out (including deleting and renaming the plugins to the single version in installedplugins.txt). however the problem still persists and seems to require manual uninstallation and reinstallation as shown by the error. I should add, only the DB was upgraded from 3.4, everything else is latest code (i.e. 4.0)

I'm having the same problem. I followed the upgrade procedures going from 3.80 to 4.00.  Tax providers are there, everything is configured but my site has 0% tax every time.  In fact even the label for tax has "Tax 0%" in it.

Andrei this is a problem - if I started orders in Texas today it will not charge tax and I will have to call all the customers. This is a horrible customer experience.

My solution was to "uninstall" the plugin and then "reinstall" it. I then had to re-setup the tax rates.

I had to do the same thing.  The weird thing was I reconfigured and was testing and things weren't working. I went in and the tax configurations were gone. Not sure how that happened. I re-added them (again) and it seems to be working now. Hopefully they don't get removed again.
