Create Topic Page

I need to create a new topic page but i need it to have labels & Text boxes &  buttons. It is like a form that the client should fill and when he clicks on the button, it should be saved in the DB so the admin can see it. How can i do it using topic page or shall i use something else?

No, topic page does not support this feature. :)

Another option is to use https://www.formsite.com/, https://www.cognitoforms.com/ or similar online form builder. You create a topic and add JavaScript code generated by FormSite to show the form.

You can also use Google Forms, and embed your forms to your website.

I have embedded forms into my topic pages which I created on this site:

Works fine..

I am able now to add a link to the home page, this link will open a custom page with my form. Now my issue is how to add this link under "Help" group (for example under the Contact Us link). I am not able to find the widget zone for this
