I'm trying to set up the Beanstream payment plugin, against a Beanstream Developer account, and Nop isn't seeing any responses back from Beanstream - either successful or failed for the transactions.
The instructions on the config page for the plug-in for handling responses is simply:

7. Complete the fields Approval Redirect and Decline Redirect of Transaction Response Pages with the URL, which might be something like https://www.yourSiteName.com/Plugins/PaymentBeanstream/ResultHandler.
8. Still on administration > account settings > order settings, complete the field Payment Gateway of Response Notification with the URL, which might be something like https://www.yourSiteName.com/Plugins/PaymentBeanstream/ResponseNotificationHandler.

I've done both. (And yes I've replaced the "yourSiteName.com" with my actual URL).
Note: If you hit those URL's with what Beanstream is sending to it, you get this page:
