Error - Manual (Fixed or By Country/State/Zip) - Address is not set

Go to Settings->Tax Settings

Middle of the page is 'Default tax settings' set any valid address there to use as default.  In my case the log soooooooo full it was causing problems.

Ya, my default tax address has been saved in Tax settings for several years, but it suddenly doesn't think we have one..the problem isn't happening on our 4.1 development site, so we'll be upgrading soon...
