Problem with Reviews

In my production environment, every time someone tries to do an review, it shows the message that there is already an review done and it wasn't. Does anyone have any tips to resolve this?

Could you provide a bit more details, steps to reproduce the problem, error logs, your configuration, and used themes/plugins?
Product reviews work fine on a clean 4.40 installation.
