Conditional handling surcharge

I need to include an additional handling charge when the order subtotal is below a minimum amount.

I also wish to have the additional handling charge displayed in the shopping cart.

It should look something like this

Sub-total:      $40.00
Handling:           8.00
Shipping:   calculated during checkout
Tax                    x.xx
Total              $xx.xx

Orders below $75.00 have a handling surcharge of $8.00

All shipping will be via UPS and I can configure shipping with an additional charge, but it's non-conditional and that charge is not listed as a separate item.
