Need Add-in Order Form (New Custom Page) - Finish within May 2017

Hi all,
I need an Order Form for our website (using Nop Commerce Version 3.4) included:

- Customer Name
- Customer Address
- Customer Mobile number

- Product
- Product details
- Price
- Promotion price (Input)
- Sub-total
- Shipment fee (Input)
- Total
- Discount (%, input)

Customer can order using this form instead of Click on Product Image, then Proceed 6 steps Check-out.
