How can we add dynamic fields in register page through code?

Yes I have tried..but that is not what I want exactly.

I want fields in the form:-
Name [TextBox]      Age  [TextBox]    Gender    [DropDown]    Relation  [DropDown]

And it should be dynamic ..that I have to add multiple rows like this.
