Manual Processing Not Showing Credit Card Information

Hi,
I am trying to setup nopcommerce v1.60 and have Manual Processing configured in the Payment Methods but when I checkout it does not prompt the customer for Credit Card Information. I have set Manual Processing and Payment Order as active.

Please help out on this issue as I have gone through the manual several times but nothing seems to fix the issue.
