Remove Product Page Form

Can someone please tell me how to remove this form that is displayed on all of my product pages?

Hi glenn,

when you created the product, you ticked the checkbox   'is gift card'


to change that now, you should select the product you need to edit ( in admin  catalog-->products-->manage products )

click the tab  'product variant sku's'  then click  view  ( unnamed product variant )

locate the offending checkbox and untick it

save
