Add Customers Own Shipping Account Details at Checkout (FedEx/UPS)

Hi there,

I am wondering if it is possible to have a shipping option whereby customers can select it and fill in their own FedEx/UPS Account details?

Currently I have a form field setup in Shipping Attributes that they can fill-in at the shopping cart level, and a radio button option labelled "Use My Own FedEx or UPS Shipping Account ($0.00)" in Shipping Methods .

This is obviously not great in terms of UX and was wondering if someone knew a better way to do this. (Keep it simple guys and girls, I'm just the font-end guy.

We've created a plugin for  Shipping.CustomerAccount, but it was done for 3.90.  It would need to be downgraded/tested for "Version 3.40".   If interested, contact us at support at noptools.com.
