FedEx, prevent International Shipping

I need to allow only FedEx for domestic services and only USPS for international services only. In the USPS configuration screen you can select that you want to disable International Carrier services, but how do I prevent International Services for FedEx? I do not have any of the 'FedEx International XXXX' selected in the FedEx services offered, yet the system allowed the following address to order using FedEx Ground from our zip code of 57106.

(Address number and name changed, billing and shipping addresses identical.)

The rate type selections in the carrier plugins just tell the plugin to "filter" the results returned by the carrier API.  The carrier API is apparently returning an option even though it may not apply - it's possible the API also returns a warning message, but the plugin may be ignoring it.
