Allow customer to designate billing vs shipping addresses, and indicate a default for each. E.g. ... add fields to Address table

ShippingAllowed
IsShippingDefault
BillingAllowed
IsBillingDefault

This would be nice for future "One Click" check out :)

This approach was used in 1.X versions, but removed in 2.X
