Shipping flat rate & in store pick up

Hi - I'm using version 1.6 and am having trouble figuring out how to configure the shipping to be free for in store pick up but a flat rate of $5 for ground shipping.  I can get all shipping to be zero or 5, and can get two options of pricing for ground and in store but not the configuration that I want.  
Any advice for this beginner is greatly appreciated!

Hi -  Me again - anyone have any suggestions for this?  I've definitely hit a brick wall.  Even considered using a discount but that didn't work.  Have been through the demo site a bunch of times - but being version 1.6 it's not much help.
