How to insert data to Order Entity

Hi,

Am New in MVC Application I Need to insert Data Order Entity During Checkout Process... I Added two fields in to the order table by changing db Also Change Order.cs and OrderMap.cs , Now I Need to Insert data into the entity from the key board. so where i can place textbox for typing data insert data?
Please Help me to go Ahead please.....
