(Maybe ITextSharp is using a temporary file, and it needs to be located in a more 'local' folder than the file share)

We have faced the same issue for a 3.90 site hosted on Azure, and an app restart has resolved the same. Did any one find a fix for this that make sure this doesn't happen again?
