Contact Us Form Error

I am using version 3.9 and when the Contact Us form is submitted it reads in the FROM line the [Store Name Online Order] where it should read [Store Name Contact Us] or whatever we type in the Subject line of the setup page of the form. How can I get the Contact Us from to read "Store Name Contact Us Form" rather than [Store Name Online Order]?

I found the issue. It was in the way I was naming my store. I do appreciate your reply. Thank you but I got it figured out.
