Credit Card Form not displayed - AuthNet PaymentModule.ascx is not called

I am unsure how, but now on Payment Information (in the checkout process), the credit card form is no longer displayed, just the "Next" button.


1) Auth.NET is the only active payment method
2) One page check disabled
3) I only have 1 shipping option and 1 payment method enabled. I deleted the other shipping options.
4) There are no country restrictions.


When I toss break points in AuthNET PaymentModule.ascx - nothing.

Does not appear that it is being called once I get to CheckoutPaymentInfo.aspx


I feel like I've exhausted things to try.. Hoping someone else has some input. frustrating, this worked before - i feel like a possible payment/shipping change may have done this, but I have no idea.

Presume that you modified 'User template path' of Authorize.NET payment module. Go to your payment module details page and ensure the specified values are the same as here
